When you visit one of our locations, a member of our billing staff works with you. Our staff have years of experience and will help answer questions that you may have about your bill.
We accept most
We work with almost every major insurer. To check out your individual benefits, you can call the number on the back of your insurance card and give them our National Provider ID (NPI) - 1073626081.
Paying your bill
We provide multiple ways for our patients to easily pay for their medical services. Our online portal allows for patients to easily schedule payments using a credit card. We also accept cheques.
At Physicians' Diagnostics & Rehabilitation, we realize your health care experience is not limited to just a clinic visit or therapy appointment. Our billing department will work with you every step of the way.
PDR Clinics is an approved provider for most major insurance carriers and health plans. Most workers’ comp, auto and personal injury insurance plans also cover services provided at PDR Clinics. We are also a preferred provider for HealthPartners and Preferred One, which may offer additional care benefits.

Our billing process
After receiving care at PDR, we will send a claim to your insurance company to pay your bill. We will do everything we can do to minimize your financial burden by maximizing your insurance benefits. You will receive monthly billing statements regarding any outstanding balance.

|1. PDR Clinics processes your account based on information provided when you scheduled. We then bill your insurance company(s) based on the care you received at our clinic.||When scheduling, please provide your most current insurance information and completed medical history forms.|
|2. Throughout your program you will receive billing statements from PDR (i.e., co-payment, co-insurance, total balance due).||Review the statement to ensure the personal and insurance information is correct. If there are any changes needed, please call 952-908-2700.
Keep your billing statements for your records. The billing statements may be helpful when reviewing the explanation of benefits (EOB) that you may receive from your insurance company.
|3. PDR will wait for payment from your insurance company. If payment is not received in a reasonable amount of time, we may contact your insurance company again.||Your insurance company may ask you to provide more information. If so, please respond quickly, and let us know the information you've provided to the insurance company, so we can update our records.|
|4. If there is a problem with processing your claim(s), PDR may request assistance to resolve the issue.||If you do not receive an explanation of benefits (EOB) from your insurance company within 30 days, please contact them to find out the status of your claim.|
|5. After your insurance company has made a payment, PDR may send you a billing statement for the remaining balance.||When you receive a billing statement, please pay your bill promptly.|
Frequently Asked Billing Questions
I received a billing statement. How do I know my insurance company paid its portion?
If you have any questions about your insurance payment, please call your insurance company directly. The insurance company's phone number is usually printed on the back of your insurance card.
When do I pay the co-payment?
Your co-payment is due at the time of your appointment. If you are unsure of your co-pay responsibilities, please look at your insurance card, or call your insurance company.
Why are some of my bills from PDR covered by my insurance, while others are not covered?
Benefit plans and coverage can change every year, and will determine whether or not a patient's bill is covered by insurance. It is not uncommon for healthcare coverage to have a deductible or out-of-pocket expense. Since each plan can be different, we encourage you to carefully review your benefits with your insurance company.
